Collaborative Assessment Transition Planning!

The ultimate goal is aimed at helping students, family members, educators, vocational rehabilitation (VR) counselors, human services and health agency staff, and other partners develop a coordinated assessment approach for transition planning and service delivery as required by the Individuals with Disabilities Education Act (IDEA, 2004) and the Rehabilitation Act of 1973 (Rehab Act).

Positive Personal Profile (PPP)

We use the PPP to collectively gather information, including assessments, observations, interviews, and discussions with the students, so student can clearly outline their strengths, interests, goals, and identify any areas where they may need support or accommodations.

Transition Assessment Services

Goal 1

Provide the opportunity for course selection, informed instructional strategies, and identify necessary accommodations to access instruction. 
 

Provide self-knowledge of their strengths and interests needed as they develop college and career readiness skills and engage in opportunities to expand career development and college preparation.

Goal 2

  • Provide support students so that they can better understand their social emotional, personal engagement, and communication skills that is needed for successful relationships and community engagement.

Goal 3

Provide assistance to students that will help them identify the transition services they need in order to reach their MPG’s in employment, postsecondary education and training, and daily living. 


Provide direct instruction to the student so that they understand the connection between their individual
academic program and post-school ambitions as a way of motivating students to stay in school.
